Decade after decade popular fads come and go flavoring our culture with warm nostalgic memories of the past. Just the mention of lava lamps, juke boxes, rubik's cubes or the electric slide can evoke feelings of either fondness or displeasure depending on your taste. While fads are often short-lived and relatively harmless, there is one type of fad that you might wish to approach with caution. Any fad in the medical industry should be carefully scrutinized because the risks could be too costly. Whether it's a fad diet or the latest new medical procedure, too often these types of newsworthy breakthroughs are reported long before they have been properly substantiated in clinical proven tests.
In the hair transplant industry there have been numerous fads and trends that fizzled out after just a few months or years. An endless list of topical crèmes, shampoos and vitamins have left hair loss patients with less hair than ever. Ineffective medical fads aren't only on the store shelves. They can be found in the operating room as well. What many hopeful patients don't know is that often times, new medical procedures become popular long before they have been properly tested and validated.
For example, many years ago hair restoration surgeons began using laser technology to create recipient sites in the balding area of the scalp where hair follicles would be transplanted. The claim was that laser technology would cause less trauma, speed up healing time and improve hair placement. In reality, the laser actually burned the scalp and cauterized delicate blood vessels beneath the surface of the skin. This caused diminished blood supply to the newly transplanted hair grafts which negatively impacted the growth and overall success of the transplant. Unfortunately, the only real casualty of this trend was the unknowing hair transplant patients who believed the media hype and lost money on a botched procedure. While the laser fad ran its course, reputable hair transplant surgeons were often left to explain why they weren't using the latest state of the art techniques in their practices.
Many hair restoration surgeons believe that Follicular Unit Extraction (FUE) is the latest fad in the hair transplant industry and nothing more than a clever marketing ploy. In a traditional hair transplant surgery, donor hair is harvested from a thin strip of tissue taken from the DHT resistant safe zone in the back of the head. This leaves a horizontal scar that may or may not be detectable depending on the skill of the surgeon. FUE is not a strip procedure which means there is no linear scar. A scarless surgery is the supposed advantage of FUE however it does sometimes leave polka dot like scars in the donor area. With FUE, follicles are harvested using a 1mm punch biopsy instrument that punches out circular grafts to be transplanted in the alopecic area. This instrument is an evolution of 4mm punch biopsy instruments that were used in unsightly doll plug style hair transplants in 80's and 90's.
The second factor that patients need to consider is the health of the hair follicle. As grafts are punched and then pulled out of the donor area, delicate hair follicles can potentially be damaged because roots beneath the skin don't line up with hairs above the surface. The base of the hair can be nipped in the process and significant transection or damage can occur. Even if the root isn't nipped there is a possibility it could be ripped during extraction. Because every patient is different, the amount of force needed to extract the graft varies from patient to patient. Some patients are more susceptible to fragmented follicles even when care is taken to remove the graft. Doctors 'should' administer a FOX test to determine if the patient is a good candidate for the procedure. During a FOX test, sample grafts are harvested and examined to measure successful harvesting. If patients receive a negative FOX test score they should not even consider getting an FUE surgery.
Another negative factor of FUE is sparse density. With FUE, punch graft harvesting is not as efficient as strip harvesting because fewer grafts survive. In a strip harvesting procedure (FUT), technicians separate grafts under magnification with precision so there is no transection or damage to the follicles. An abundance of healthy follicles results in more hair and better results. If hairs survive the violent removal in an FUE surgery, they are typically one and two hair grafts only. These small grafts do not deliver density to areas behind the hairline and actually doom the patient to a sparse result. A typical FUE procedure transplants approximately 2000 hairs. A well done FUT procedure can yield as many as 7000 hairs. Besides less hair to the recipient area, with FUE the back of the head is robbed of healthy hair follicles leaving the area visibly thinned. With a traditional FUT procedure the donor strip is completely removed so there is noh evidence of harvesting.
Finally, the other consideration is the cost of the surgery. Although an FUE surgery requires less staff, doctors choosing to do FUE surgery often times double their revenue. Hopeful patients are willing to pay the price because FUE is highly publicized as a breakthrough in the hair restoration industry.
